Requirements vary state to state, but the paperwork problem is universal. Here's what you're almost always going to need:
Every state requires proof of vaccination for school enrollment. Most need the specific dates of each dose — "up to date" isn't enough. Check your pediatrician's portal or your state immunization registry (most have a parent portal online).
Most states require a health examination at certain grade levels (kindergarten entry is nearly universal). Some states accept exams up to 12 months old; others require a current-year exam. Your state page above has the specifics.
Generic records aren't always enough. Many schools and districts have specific forms that your child's physician needs to sign. Call the school office or check their website before the pediatrician appointment, so you bring the right paperwork.
If your child carries an EpiPen, inhaler, or takes medication at school, you'll need a new action plan signed by a physician for this school year. Last year's plan usually doesn't carry over.
