Delaware school immunization requirements are set under 14 Del. C. §131 and 16 Del. Admin. Code §4202, administered by the Delaware Division of Public Health (DPH). Whether your child is starting kindergarten at Red Clay Consolidated Schools, entering 7th grade at Christina School District, or transferring into a Capital School District building, here is every required form, where to get records, and the grade-specific details.

The Checklist

1. Delaware certificate of immunization

Under 14 Del. C. §131 and 16 Del. Admin. Code §4202, a certificate of immunization is required for school enrollment in Delaware. The certificate is issued by a licensed healthcare provider or generated from the Delaware Immunization System (DELPHI).

Required vaccines for Delaware school entry:

DTaP — 5 doses (or 4 doses if 4th dose at age 4+)
IPV — 4 doses
MMR — 2 doses
Hepatitis B — 3 doses
Varicella — 2 doses (or documented immunity)
Hepatitis A — 2 doses (required for kindergarten entry)
Tdap booster — required at 7th grade entry
MenACWY — required at 7th grade entry

Hepatitis A is required for kindergarten entry in Delaware. Families relocating from states that do not include Hep A in their school requirements should confirm the 2-dose series is complete before registering at Red Clay, Christina, Brandywine, or any Delaware school.

2. DELPHI — Delaware's immunization registry

The Delaware Immunization System (DELPHI), managed by the Delaware Division of Public Health, tracks immunization records for children vaccinated in Delaware. Your child's provider can access DELPHI and print a current immunization history for school enrollment.

To get your child's Delaware immunization record:

Ask your child's primary care provider — they can access DELPHI and print a current certificate.
Contact the Delaware Division of Public Health for record request guidance.
For out-of-state transfers, bring all prior vaccination documentation to the first Delaware provider appointment so records can be entered into DELPHI.

3. Grade-specific checkpoints

Kindergarten entry

Full immunization certificate required including the Hep A 2-dose series. Red Clay Consolidated and Christina School District require documentation at time of enrollment. Delaware allows provisional enrollment for families working to complete documentation — contact your district to confirm the timeline and next steps.

7th grade entry

Tdap booster and first dose of MenACWY required at 7th grade entry. Schedule these in spring to have the updated DELPHI-verified record ready before fall registration at Brandywine, Capital, Indian River, or other Delaware districts.

All

Transfer students

Out-of-state transfers must meet Delaware's requirements. Hep A is the most commonly missed vaccine for students transferring from states that don't require it. A DELPHI-verified or provider-issued Delaware certificate is required.

Exemptions

Delaware immunization exemptions

Delaware provides two types of exemptions from school immunization requirements under 16 Del. Admin. Code §4202:

Medical exemption — written statement from a licensed Delaware physician certifying that a specific immunization is medically contraindicated for the child.
Religious exemption — written statement from a parent or guardian that immunization conflicts with their sincere religious beliefs.
