Pennsylvania DHS regulations (55 Pa. Code Chapter 3270) require a Child Health Assessment for every child entering a licensed child care center. The assessment must be completed by a licensed physician, CRNP, or physician assistant.
The initial health assessment must be on file within 60 days of enrollment. After that, an updated health report is required every 12 months for children under age 2, and every 24 months for children ages 2 and older.
The health assessment includes a physical examination, developmental screening, lead screening (for children 9–12 months and again at age 2), and documentation of allergies, dietary restrictions, and any special health needs.
