Ohio Revised Code §3313.671 requires every student to present proof of immunization within 14 days of starting school. If the record isn't submitted within 14 days, the student may be excluded until compliance.
Required vaccines for school entry (K–12):
Ohio schools conduct required screenings under ORC §3313.69 and ORC §3313.673. These are done at school — you don't need to schedule them separately, but knowing about them helps you follow up if something is flagged.
Ohio does not require a comprehensive physical exam for general school entry at the state level.
