California licensed childcare facilities and many camps require immunization records, a physician's report, and sometimes a TB risk assessment — before your child's first day. Here's every form, who fills it out, and where to get it.

California Health & Safety Code §1596.798 requires all children in licensed childcare to have age-appropriate immunizations documented on the CDPH 286 (the same "Blue Card" used for school). The child's immunizations must be up to date for their age at enrollment — including doses due earlier than the school-entry schedule.
Key childcare immunizations (age-appropriate, per CDC schedule):
California Community Care Licensing requires a Physician's Report (LIC 701) for every child entering licensed childcare. This form must be completed by a physician within 30 days before or 30 days after enrollment.
The LIC 701 includes a physical exam, health history, and a determination that the child is healthy enough for group childcare. It also includes a TB risk assessment — the physician checks whether TB testing is recommended based on the child's risk factors.
The form is valid until the child's next scheduled well-child exam, and must be updated annually for children in ongoing care.

California day camps operated by licensed childcare facilities follow the same rules as daycare (CDPH 286 + LIC 701). Camps run by schools, parks departments, or private organizations may have different requirements:
