Pennsylvania requires proof of immunization for all students attending public, private, parochial, or home education programs. The requirements are set by the PA Department of Health under 28 Pa. Code §23.83–23.86 and vary by grade level.

Pennsylvania's School Health Law (24 P.S. §14-1402) requires a comprehensive physical examination at three points:
Students new to the district also need a physical exam upon enrollment. The exam can be completed by the child's private physician within the year prior to the grade in which the exam is required. If the family doesn't have a physician, the school medical provider conducts the exam.
Schools provide a health examination form — bring it to the pediatrician appointment so results are recorded in the format the school requires.

Pennsylvania requires a dental examination at three grade levels (24 P.S. §14-1403):
The exam must be performed by a licensed dentist. If completed by a private dentist, the results should be submitted on the school's dental examination form. If the family cannot obtain a private dental exam, the school dental hygienist performs one.
