Florida Statute §402.305 requires all children attending licensed childcare to have a current DH 680 (Florida Certification of Immunization) on file. This form documents age-appropriate vaccines and must be completed by a Florida-licensed healthcare provider or county health department.

Florida requires a Student Health Examination (DH 3040) for children entering childcare, pre-K, or school for the first time. The exam must be completed within 12 months before enrollment.
The DH 3040 includes a comprehensive physical exam, growth measurements, vision and hearing screening, and a review of health history. It must be completed by a physician, advanced practice nurse, or physician assistant licensed in Florida.
County health departments offer low-cost or free school and childcare physicals if your child doesn't have a regular pediatrician.
