The DH 680 (Florida Certification of Immunization) is the state's official proof-of-vaccination form. Every child entering a Florida school — public, private, or parochial — must have a current DH 680 on file. The form is completed and signed by a Florida-licensed physician, county health department, or authorized healthcare provider.

If your child has allergies requiring an EpiPen, asthma requiring an inhaler, or takes any medication during school hours, Florida law (§1002.20(3)(h)) allows self-carry and self-administration with a physician-signed medication action plan on file with the school. Each school district has its own medication administration form — request it from the school nurse before the first day.
