Invite a patient to start their record
The family owns the record from the first minute; you never hold it. When they start it, you join the care team as a viewer and we email you the record's private inbox address so the first visit summary can go in the same day.
Sign in with a free Family Stork account to send invitations. It takes a minute and is only used so patients can see who invited them.
How to use it, in three steps
- Invite the patient. Use the form above (your name, your clinic, the patient or parent's email). They get one email that names you. Already using Family Stork? Skip this: ask the family for the record's private email address instead.
- They start the record. Free, about a minute, no card. The record is theirs from the first minute. The moment it exists we email you its private address, something like
emma-x7k2…@families.familystork.com, and you are on the care team as a viewer. - Reply to that address with the paperwork. Visit summaries, lab reports, immunization records, referral letters, bills. PDF or a photo, as attachments, from any email program. Each one files itself into the patient's timeline with your clinic's name and contact read from the letterhead. You get a confirmation reply, like a fax.
For the front desk: the address is per patient. Put it in the patient's chart next to their email and CC it whenever you send them paperwork. Nothing to log into.

Sending records
Every patient has a private Family Stork email address. Reply to it with attachments, like visit summaries, labs, and immunization history, and they file into the family's record. You get a confirmation reply, like a fax. Your first email waits for the family's approval; after that your clinic is trusted.
Every email does two favors for your patient. Your reports file themselves into an organized timeline the family can actually find. And your clinic's name, phone, and address land on the patient's record and emergency card automatically, read from your letterhead.
Reading the record
Families can hand you a one-page printable summary: problem list, current medications, allergies, latest labs, immunization status, family health history, and contact. Clinical voice only. No AI conclusions.
And because families can connect supported health portals like MyChart, email records in, and upload what they already have, the summary in front of you is the whole cross-provider picture — every clinic, hospital, and specialist in one place.
If you join a care team
Optional, always. You can view everything, ask the record questions, and upload. You can never edit or delete, and every access is logged. The family can remove you at any time.
The line we do not cross
Family Stork organizes the record and helps families prepare better questions. Doctors diagnose and treat. The AI never diagnoses, never triages, and it tells families to call you when something sounds urgent. Every answer it gives shows the record it came from.
