Daycare Requirements

1. Immunization records for licensed child care

Under Wyo. Stat. §21-4-309, all children enrolled in a Wyoming licensed child care facility must have documented proof of age-appropriate immunizations. The Wyoming Department of Health maintains the Wyoming Immunization Registry (WyIR). Parents can also access their child's WyIR record through the Docket app.

Required vaccines for Wyoming child care entry (age-appropriate schedule):

DTaP — age-appropriate doses per CDC schedule
IPV (Polio) — age-appropriate doses
MMR — 1st dose at 12 months; 2nd dose before kindergarten
Hepatitis B — 3 doses, series started at birth
Varicella — 1st dose at 12 months
Hib — age-appropriate doses
PCV13 (Pneumococcal) — age-appropriate doses

Wyoming allows provisional enrollment for children who are in the process of completing their immunization series, provided a written schedule for completion is on file. Your child's provider can confirm age-appropriate status and provide a WyIR printout or Docket verification.

2. Health and enrollment forms

Wyoming DFS licensing rules require licensed child care facilities to collect health information at enrollment. Most Wyoming child care programs will ask for:

Child enrollment/health history form — covering allergies, chronic conditions, medications, and emergency contacts
Immunization certificate — WyIR printout, Docket verification, or provider-signed record
Allergy action plan — if the child has food allergies, insect sting allergies, or asthma, a signed action plan from the prescribing provider is typically required
Medication administration authorization — for any daily or as-needed medications

Wyoming does not mandate a separate physician-signed physical exam for child care entry at the state level, but individual programs may request one. Check with your specific program.

3. Summer camp health forms

Wyoming summer camps require health documentation before the session starts. Requirements vary by camp type:

Licensed day camps follow Wyoming DFS child care licensing requirements: immunization records and a health history on file.
Residential (overnight) camps typically require a more detailed health form covering medications, allergies, swimming ability, behavioral health notes, and authorization for emergency medical treatment. Many Wyoming residential camps use the ACA health form or their own equivalent.
Ranch and adventure camps — Wyoming has a strong tradition of ranch camps and wilderness programs, especially in the Teton, Bighorn, and Wind River areas. These often require a physical exam within 12 months and a signed assumption-of-risk form covering horseback riding, hiking, and outdoor activities.

Wyoming's camp season runs June through August. Forms are typically due 2–4 weeks before the session — check your camp's specific deadline.
