Under federal law (HIPAA Right of Access), your previous providers must release your child's medical records upon written request. Start this process at least 2–4 weeks before your move date.
Utah's immunization registry, USIIS (Utah Statewide Immunization Information System), is maintained by the Utah Department of Health and Human Services. Your child's out-of-state immunization records will not automatically appear in USIIS — you must bring them to a Utah provider to have them entered.
At your child's first Utah well-child visit, bring the official immunization certificate from your previous state. The new provider will enter the doses into USIIS.
After the visit, log into MyUtah and navigate to Health → Immunization Records. Confirm all historical doses appear correctly. Out-of-state entries are sometimes incomplete — verify dose counts against your original records.
Once verified, print the official USIIS immunization record — this is the document Utah schools accept for enrollment under Utah Code §53G-9-303.
