These grade-7 requirements are separate from the childhood series. A student who completed all kindergarten requirements still needs Tdap and MenACWY documented before the first day of 7th grade.
USIIS (Utah Statewide Immunization Information System) is the statewide immunization registry maintained by the Utah Department of Health and Human Services. Most Utah healthcare providers report administered vaccines to USIIS.
Parents can view and print their child's immunization record through the MyUtah portal. Create an account if you don't already have one, then navigate to Health → Immunization Records.
Your child's pediatrician can print a USIIS summary at any visit. If doses are missing from the registry (common for vaccines given out of state), bring the original records to your provider to update USIIS.
Utah local health departments can access USIIS and provide an official immunization record. Salt Lake County Health Department, Utah County Health Department, and Davis County Health Department each offer walk-in record access.
If your child is missing required doses, Utah allows conditional enrollment — the child can attend school while completing a documented catch-up schedule.
