Florida Statute §1003.22 sets the health requirements for school enrollment. Florida uses two specific state-issued forms that your child's new school will ask for:

Florida SHOTS (State Health Online Tracking System) is Florida's statewide immunization registry. Florida providers report doses to Florida SHOTS. Families can access their child's Florida SHOTS record through the Florida Health website — the lookup is free. If your child has received any vaccinations in Florida (for example, on a prior visit or move), those records may already be there.
For doses administered outside Florida, your previous state's immunization registry printout or a signed summary from your former pediatrician is the starting point. Bring these to your first Florida pediatrician appointment — they can enter out-of-state doses into Florida SHOTS and complete the DH 680 at the same visit.

Florida Statute §1002.22 requires a student's previous school to forward student records to a requesting Florida school within 5 school days of receiving a written request. This is one of the fastest transfer windows in the country. Your new Florida school handles the request — you just need to tell them the previous school's name and address at enrollment.

Florida's DH 680 must be signed by a Florida-licensed healthcare practitioner — your out-of-state pediatrician cannot complete this form. Schedule a first appointment quickly after arriving. Florida pediatric practices frequently see newly arrived families and are accustomed to completing DH 680 from out-of-state records at the first visit.
Bring to your first appointment:
Your new Florida pediatrician will use this to complete the DH 680, update Florida SHOTS with any missing doses, and provide a school health exam if the current one is outside the 12-month window.
Get a signed immunization history from every provider your child has seen. Request physical exam notes and specialist records. Florida's DH 680 requirement means a Florida provider will need all of this — don't arrive with just a verbal account.
Florida schools allow a grace period for DH 680 while you find a provider. Enroll immediately. The school initiates the prior-school records request (5-day window under §1002.22). You're on the clock for finding a Florida pediatrician — but you have time.
Bring your records package. Your new Florida provider completes the DH 680, enters doses into Florida SHOTS, and provides any school forms your district needs. Deliver the DH 680 to the school and enrollment is complete.
