Moving to Delaware:
your family's medical records checklist.

Delaware sits between Philadelphia, Baltimore, and the DC metro — which means transfers from neighboring states are especially common. Here's what Delaware requires for school enrollment, how to get records through DELPHI, and how to handle the transition whether you're moving to the Red Clay, Christina, or Capital school district.

Required at Enrollment

What Delaware schools need on day one

14 Del. C. §131 and 16 Del. Admin. Code §4202 require proof of immunization for school entry. Your child's new Delaware school will ask for a certificate of immunization showing dated dose records for DTaP, IPV, MMR, Hepatitis B, Varicella, and Hepatitis A (kindergarten) — plus Tdap and MenACWY at 7th grade.

What Delaware needs, specifically: Certificate of immunization (provider-signed or DELPHI printout) · Physical exam (many districts require one within the past year for new enrollees) · Prior school's cumulative records · Birth certificate or proof of age · Proof of Delaware residency · Any current IEP or 504 plan documentation

Three counties, one set of rules

Delaware has only three counties — New Castle (Wilmington, Newark), Kent (Dover), and Sussex (Rehoboth, Georgetown). All follow the same state immunization requirements. District-level enrollment processes vary, but the vaccine documentation is identical statewide.

How the old school's records transfer

Your new Delaware school will request cumulative records from your child's former district. Delaware law requires schools to maintain and share student records upon transfer request. Processing varies — typically 2–4 weeks from out of state. Bring your own copies to the enrollment appointment to avoid waiting on the mail.

Immunization Records

Getting your child's vaccine history into Delaware's system

Once you have a new Delaware provider, they can enter out-of-state doses into DELPHI — the Delaware Immunization System. Contact your child's former pediatrician for a signed immunization summary first — this is the fastest path.

If your family used providers in neighboring states (PA, MD, NJ), those doses may already be in the sending state's registry. Request a printout and bring it to your new Delaware provider for verification and entry into DELPHI.

Cross-state providers

Delaware families frequently see providers across state lines — a pediatrician in Philadelphia, a specialist at Johns Hopkins in Baltimore, or a dentist in Cherry Hill. Doses administered out of state won't automatically appear in DELPHI. When establishing care with a Delaware provider, bring records from every provider, regardless of which state they're in.

Your new provider will enter records into DELPHI going forward. Out-of-state doses can be added retroactively with dated documentation.

District-Specific Notes

Red Clay, Christina, and Capital enrollment

Red Clay Consolidated School District — Serves much of northern New Castle County including parts of Wilmington. Enrollment is handled at individual school offices. Bring immunization documentation, proof of residence, and birth certificate.

Christina School District — Covers Newark, Bear, and parts of Wilmington. The district's Choice & Enrollment Office processes transfers. Christina follows the state's provisional enrollment rules for students with in-progress immunization series.

Capital School District — Based in Dover (Kent County). The Central Registration Office handles enrollment. Bring all health documentation to the registration appointment — Capital processes new enrollees year-round.

Your Moving Timeline

What to do, and when

1

Before you leave: gather everything

Request a complete medical record summary from every current provider — including any cross-state providers. Ask for a signed immunization history. If your child hasn't completed the Hepatitis A series and is approaching kindergarten age, ask your current provider to start it before the move.

2

Week 1 in Delaware: enroll, start the transfer

Enroll at the assigned school and bring immunization records, proof of residence, and birth certificate. Ask the school to request cumulative records from the former district. Delaware allows provisional enrollment while the series is being completed on schedule.

3

Find a new pediatrician and complete the certificate

Your new Delaware pediatrician will review existing records, enter doses into DELPHI, and provide the immunization certificate the school needs. If you plan to continue seeing providers across state lines, bring each visit's records to your Delaware provider for DELPHI entry.

The Simpler Way

One email address that moves with you

Every person gets one health story, and one private Family Stork email address every clinic already knows how to use. Tired of managing health records for yourself and everyone you love? Family Stork organizes them for you automatically — no portal logins, no uploads, nothing for any clinic to learn.

Now that you're in Delaware: Check the Delaware school health form checklist for every form, deadline, and physician-signature requirement your new school will ask for. Need daycare or camp forms too? See Delaware daycare and camp health forms.
