The Checklist

1. New Mexico immunization certificate

Under NMAC 7.4.2, any child enrolling in a New Mexico school must provide a completed immunization certificate issued by a licensed healthcare provider, a public health office, or verified through NMSIIS (New Mexico State Immunization Information System). No child may be admitted without the required documentation or an approved exemption.

Required vaccines for New Mexico school entry:
  • DTaP — 5 doses (or 4 doses if 4th dose at age 4+)
  • IPV — 4 doses
  • MMR — 2 doses
  • Hepatitis B — 3 doses
  • Varicella — 2 doses (or documented immunity)
  • Hepatitis A — 2 doses (kindergarten entry requirement)
  • Tdap booster — required at 7th grade entry
  • MenACWY — required at 7th grade entry

Hepatitis A is required for New Mexico kindergarten entry. Families moving from states that do not require Hep A for school entry should verify the 2-dose series is complete before registering at Albuquerque Public Schools (APS) or any NM district.

2. NMSIIS — New Mexico's immunization registry

The New Mexico State Immunization Information System (NMSIIS), managed by NMDOH, tracks immunization records for children vaccinated in New Mexico. Your child's provider can access NMSIIS and print a current history for school enrollment.

3. Grade-specific checkpoints

Kindergarten entry

Full immunization certificate required, including the Hep A 2-dose series, at time of enrollment. New Mexico allows a provisional enrollment period for families with incomplete documentation — contact your school or district office to confirm the timeframe and required next steps.

7th grade entry

Tdap booster and first dose of MenACWY required. Schedule these in late spring to have the updated record ready for fall enrollment. Albuquerque Public Schools and Las Cruces Public Schools both require the updated 7th-grade certificate before the school year begins.

All

Transfer students

Out-of-state transfers must meet NM requirements — not the sending state's. Hep A is the most common gap for students from states that don't require it. NMSIIS verification or a provider-issued NM certificate is required.

4. District enrollment — Albuquerque, Las Cruces, Rio Rancho, Santa Fe
