Nebraska requires proof of age-appropriate immunizations before school entry under Neb. Rev. Stat. §79-221. The Checklist

1. Nebraska immunization certificate

Under Neb. Rev. Stat. §79-221, a school shall not admit or retain a pupil unless the pupil has been immunized as required by the Nebraska Department of Health and Human Services (DHHS). The required certificate is typically a printed record from your child's healthcare provider or from the Nebraska State Immunization Information System (NESIIS).

Required vaccines for Nebraska school entry:

DTaP — 5 doses (or 4 if 4th dose at age 4+)
IPV — 4 doses (or 3 if 3rd dose at age 4+)
MMR — 2 doses
Hepatitis B — 3 doses
Varicella — 2 doses (or documented immunity)
Hepatitis A — 2 doses (kindergarten entry requirement)
Tdap booster — required at 7th grade entry
MenACWY — required at 7th grade entry

Hepatitis A is required for kindergarten entry in Nebraska. Families relocating from states that do not include Hep A in their school requirements should confirm completion of the 2-dose series with their pediatrician before attempting enrollment in Omaha, Lincoln, or any Nebraska school.

2. NESIIS — Nebraska's immunization registry

The Nebraska State Immunization Information System (NESIIS), administered by Nebraska DHHS, tracks immunization records for children vaccinated in Nebraska. Your child's pediatrician can access NESIIS and print a current immunization history for school enrollment.

To get your child's Nebraska immunization record:

Request a printed record from your child's primary care provider — they can access NESIIS directly.
Contact your local Nebraska health department — county health departments can look up NESIIS records.
For out-of-state transfers, bring all prior vaccination documentation to the first Nebraska provider appointment so records can be entered into NESIIS.

3. Grade-specific checkpoints

Kindergarten entry

Full immunization certificate required, including the Hep A 2-dose series. Omaha Public Schools (OPS) and Lincoln Public Schools (LPS) both require the immunization record before or at enrollment. A provisional enrollment period may be available for families working to complete documentation — contact your specific school to confirm.

7th grade entry

Tdap booster (if not received after age 7) and the first dose of MenACWY are required for 7th grade entry. Schedule these immunizations in late spring or early summer so the updated record is ready before fall registration.

Transfer students

Out-of-state transfers must meet Nebraska requirements — not just the sending state's. Hep A is the most commonly missed vaccine for students transferring from states that don't require it. A NESIIS-verified or provider-issued Nebraska immunization record is required.

Grand Island Public Schools: Walk-in enrollment; immunization certificate required at time of registration.

Nebraska immunization exemptions

Nebraska law provides two types of exemptions from school immunization requirements under Neb. Rev. Stat. §79-221:

Medical exemption — signed statement from a licensed physician stating that immunization would be injurious to the health of the child.
Religious exemption — signed statement from the parent or guardian that immunization conflicts with their religious beliefs.
