Maine school health form checklist:
every form you need for the 2026–2027 school year.

Maine school immunization requirements are set under 22 M.R.S.A. §1064, administered by Maine CDC (Center for Disease Control). Maine is also one of a small number of states where only medical exemptions are recognized — religious and philosophical exemptions were eliminated by LD 798 in 2019. Whether your child is starting kindergarten in Portland, entering 7th grade in Lewiston, or transferring to a Bangor school, here is every required form and where to get records.

The Checklist

1. Maine immunization certificate

Under 22 M.R.S.A. §1064, no child may be admitted to a Maine public school without evidence of required immunizations. The certificate is typically a printed record from the child's healthcare provider or an ImmPact (Maine's immunization registry) printout.

Required vaccines for Maine school entry:

DTaP — 5 doses (or 4 doses if 4th dose at age 4+)
IPV — 4 doses
MMR — 2 doses
Hepatitis B — 3 doses
Varicella — 2 doses (or documented immunity)
Tdap booster — required at 7th grade entry
MenACWY — required at 7th grade entry

Physical exam: Maine schools require a school health assessment form (completed by a licensed healthcare provider) before or at kindergarten entry. The form documents general health status and readiness for school. Contact your child's pediatrician in late spring to schedule the kindergarten physical — appointment slots fill quickly in August.

2. ImmPact — Maine's immunization registry

ImmPact is Maine's statewide immunization registry, managed by Maine CDC. Healthcare providers who vaccinate children in Maine enter records into ImmPact. Your child's provider can access ImmPact and print a current immunization history for school enrollment.

To get your child's Maine immunization record:

Ask your child's primary care provider — they can access ImmPact and print a current immunization record.
Contact Maine CDC's ImmPact program for guidance on record requests.
For out-of-state transfers, bring all prior vaccination documentation to your first Maine provider appointment so records can be entered into ImmPact.

3. Grade-specific checkpoints

Kindergarten entry

Full immunization certificate plus school health assessment form required. Portland Public Schools and Lewiston Public Schools require both documents at the time of registration. Maine allows provisional enrollment for families working to complete documentation — contact your district to confirm the specific window.

7th grade entry

Tdap booster (if not received after age 7) and first dose of MenACWY required at 7th grade entry. Schedule these in spring so the updated immunization record is ready before fall school registration opens in August.

All

Transfer students

Out-of-state transfers must meet Maine's requirements. Maine eliminated religious and philosophical exemptions in 2019 (LD 798) — families with non-medical exemptions from other states will need to complete required immunizations or obtain a Maine-issued medical exemption.

Exemptions

Maine immunization exemptions — medical only

Since 2019, Maine recognizes only medical exemptions from school immunization requirements under 22 M.R.S.A. §1064(2), as amended by LD 798. Religious and philosophical exemptions are no longer available in Maine.

Medical exemption — written statement signed by a licensed physician, physician assistant, or nurse practitioner certifying that a specific immunization is medically contraindicated for the child, along with the specific clinical reason.
