Connecticut has some of the strictest school immunization requirements in the country. Under C.G.S. §10-204a, every child entering Connecticut public or private school must have documented immunizations — and Connecticut eliminated the religious exemption effective April 2021. Connecticut requires a Health Assessment Record (HAR-3) — a standardized physical exam form — for all students entering a Connecticut school for the first time, entering kindergarten or first grade, and entering grade 6 or 7. The HAR-3 must be completed and signed by a licensed physician, APRN, or physician assistant.

Only medical exemptions are accepted — signed by a physician, documenting a specific medical contraindication. Students who had a religious exemption on file before April 2021 may be grandfathered in for the grade span they were enrolled in at the time.
Connecticut requires two additional vaccines for students entering grade 7: a Tdap booster (if not already given at age 11+) and the first dose of meningococcal conjugate vaccine (MenACWY). These are separate from the childhood DTaP series and must be documented before the student's first day of 7th grade.
If your child is entering 7th grade this year and hasn't had their 11-year well-child visit yet, schedule it now — the Tdap and MenACWY doses are typically given at that visit. Most pediatricians will complete both the HAR-3 physical and the required vaccinations in one appointment.
Log in to CT WiZ or ask your pediatrician to print a CT WiZ summary. Verify all required doses are present, especially the Tdap and MenACWY for 7th graders.
New enrollees, kindergartners, and 6th/7th graders need the Health Assessment Record. Book 4–6 weeks ahead — late-summer slots fill fast in Connecticut.
Connecticut law requires immunization records and the HAR-3 (when applicable) on file before the child's first day. Students without documentation may be excluded from school until the records are provided. A child may attend on a temporary basis only if a catch-up schedule is documented and in progress.
