The Requirements

Immunizations Connecticut schools require — by grade

Kindergarten / first-grade entry

Grade 7 entry (additional requirements)

These grade-7 requirements are separate from the childhood series. A student who completed all kindergarten requirements still needs Tdap and MenACWY documented before the first day of 7th grade.

How to Access Records

CT WiZ — Connecticut's immunization registry

CT WiZ is Connecticut's statewide immunization information system, maintained by the Department of Public Health. Most Connecticut healthcare providers report administered vaccines to CT WiZ.

How to get your child's CT WiZ record

Access the CT WiZ public portal

Visit ctwizpublicportal.dph.ct.gov. Parents and guardians can access their minor child's immunization records directly through this portal.

Ask your pediatrician

Your child's pediatrician can print a CT WiZ summary at any visit. If doses are missing from the registry (common for vaccines given out of state), bring the original records to your provider to update CT WiZ.

Contact your local health department

Connecticut local health departments can also access CT WiZ and provide an official immunization record. This is especially useful if you've recently moved and haven't established a pediatrician yet.

What If Records Are Missing?

Gaps, catch-up, and conditional enrollment

If your child is missing required doses, Connecticut law allows conditional enrollment — the child can attend school temporarily while completing a documented catch-up schedule. The school health office will work with your pediatrician to set a timeline. However, if the schedule is not followed, the school can exclude the child until compliance is achieved.

Exemptions

Medical exemptions only

Connecticut recognizes medical exemptions only from school immunization requirements under Public Act 21-6 (effective April 28, 2021). A medical exemption requires a physician's written statement documenting a specific medical contraindication to a particular vaccine.
