what you need before enrollment.

Connecticut childcare facilities licensed by the Office of Early Childhood (OEC) require documented immunizations and a current physical exam before a child can attend. Required Documents

1. Physical exam and health assessment

Connecticut requires a current physical exam for all children enrolling in a licensed childcare program. The exam must be performed by a licensed physician, APRN, or physician assistant and must be completed within 12 months prior to enrollment. The Connecticut Health Assessment Record (or an equivalent form accepted by the facility) is the standard documentation.

Under C.G.S. §10-204a and OEC licensing regulations, Connecticut childcare centers require age-appropriate immunizations on file before enrollment. The daycare/preschool schedule includes:

Camp-specific forms vary by program. Contact your camp's health director for their specific form requirements and deadlines. Most Connecticut camps begin collecting health forms 30–60 days before the session starts.
