Code of Virginia §32.1-46 requires all children attending a licensed child care center, family day home, or certified preschool to have documented proof of age-appropriate immunizations before enrollment.
Required immunizations for child care (age-appropriate):
Virginia Department of Social Services (VDSS) child care licensing standards require a physical examination for every child within 30 days before or after enrollment in a licensed child care center. The exam must be performed by a licensed physician, nurse practitioner, or physician assistant.
The exam documents the child's general health, developmental status, and any health conditions the child care provider should be aware of. Many providers document this on Virginia's recommended child health form or on their own letterhead. If your child had a recent well-child visit, the results of that exam typically satisfy this requirement — bring a copy of the visit summary to the center.

If you've lost your child's immunization record, Virginia's VIIS (Virginia Immunization Information System) is the recovery tool. Most Virginia providers report doses to VIIS. Contact your local health department or your child's provider to request a printout. VIIS records are accepted by child care facilities as proof of immunization.
For children who received some doses outside Virginia, bring documentation of those doses so your Virginia provider can enter them into VIIS.

Licensed child care centers need an immunization record and a physical exam. Camps may have their own health forms. Call ahead and get the exact list before the doctor visit.
