Oklahoma requires a Certificate of Immunization for every student enrolling in school. Under Title 70 §1210.191, no child may attend an Oklahoma school without a current certificate or an approved exemption. The certificate is issued by a licensed physician, the local county health department, or another authorized healthcare provider.
Required vaccines on the Oklahoma Certificate of Immunization (K–12):
7th grade: Tdap booster required at or before 7th grade entry.
Oklahoma maintains the OSIIS (Oklahoma State Immunization Information System) registry, which consolidates immunization records from participating providers across the state. OSIIS records can be retrieved through your child's physician or any OSDH county health department.
If your family has recently moved to Oklahoma from another state, your child's prior records may not be in OSIIS. Bring paper vaccination records from your prior providers — your Oklahoma pediatrician can enter them and issue a current certificate based on the combined history.

Oklahoma law provides specific authority to exclude unvaccinated students during disease outbreaks. Under Title 70 §1210.191(B), if a declared outbreak of a vaccine-preventable disease occurs in a school, unvaccinated students — including those with religious or medical exemptions — may be excluded from school for the duration of the outbreak.
This is a practical consideration for families with exempted children: an outbreak in the school district can result in an extended absence from school with no obligation for the school to provide alternative instruction. Parents should be aware of this before filing an exemption.

Call your child's pediatrician or the OSDH county health department to pull the OSIIS history. Confirm all doses are on file — especially if you're moving from another state.
Your physician will complete and sign the Certificate of Immunization. If any doses are missing, they can administer them at the same visit. Book early — late August is the busiest time for Oklahoma pediatricians.
