Under federal law (HIPAA Right of Access), your previous providers must release your child's medical records to you or your new provider upon written request. Start this process at least 2–4 weeks before your move date.
Nevada's statewide immunization registry, NV WebIZ, is maintained by the Division of Public and Behavioral Health. Your child's out-of-state immunization records will not automatically appear in WebIZ — you need to bring them to a Nevada provider to have them entered.
At your child's first Nevada well-child visit, bring the official immunization certificate from your previous state. The new provider will enter the doses into NV WebIZ.
After the visit, check izrecord.nv.gov to confirm all historical doses appear correctly. Out-of-state entries are sometimes incomplete — verify dose counts against your original records.
Once verified, print the official NV WebIZ immunization certificate — this is the document Nevada schools accept for enrollment under NRS 392.435.
