Arizona law requires schools to transfer records upon request. Keep your own copies — records requests between states can take longer than within-state transfers, and having copies avoids enrollment delays.

ASIIS (Arizona State Immunization Information System) is Arizona's statewide immunization registry. If your child received any vaccines in Arizona previously, those doses are likely in ASIIS. Your new Arizona pediatrician can access ASIIS directly and print your child's record. You can also contact your county health department for a printout.
For doses given in your previous state, bring a signed immunization record from your former pediatrician. Arizona schools accept a signed physician's record listing each vaccine and date. Your former state's immunization registry may have a downloadable or printable record — request it before you move.
Arizona-specific note: Arizona requires Hepatitis A vaccination for school entry — a requirement not all states share. If your child hasn't received Hep A vaccines, your new Arizona pediatrician can administer them and update the record.
