Required Documents

1. Indiana State Department of Health Form 680

Under IC 20-34-4-2, every child enrolling in an Indiana school must provide proof of immunization. The official document is Indiana State Department of Health Form 680 (the Immunization History Form). This form must be completed and signed by a licensed physician or authorized public health provider — it cannot be self-certified by parents.

Required vaccines for K–12 school entry in Indiana:

6th grade entry: Students entering 6th grade in Indiana must have proof of a Tdap booster and a MenACWY (meningococcal) vaccine. These are grade-specific requirements distinct from the standard series.

2. Physical exam — check your district's requirements

Indiana does not mandate a physical examination for school entry at the state level. However, many Indiana school corporations have their own health form policies. Indianapolis Public Schools, Fort Wayne Community Schools, and other large districts have historically requested health screening information during kindergarten enrollment, particularly for incoming kindergarteners.

3. CHIRP and the MenACWY requirement at 6th grade — Indiana-specific milestones

Indiana's CHIRP (Children and Hoosiers Immunization Registry Program) is maintained by the Indiana Department of Health and records doses administered by participating providers across the state. If your child has received vaccines at multiple locations — a pediatrician, urgent care, a pharmacy, or a county health department — CHIRP is the most complete single view of their record.

The 6th-grade MenACWY requirement is a notable Indiana milestone. Meningococcal disease can spread in school settings, and Indiana's law requires documented vaccination before a student may enter 6th grade. If your child is making this grade transition, verify both the Tdap booster and MenACWY are on their Form 680 before the school year begins.

Your Indiana school enrollment timeline

Access CHIRP records (4–6 weeks before school)

Ask your pediatrician or the local health department to retrieve your child's CHIRP record. Confirm all doses are documented, including both Hep A doses and the 6th-grade Tdap/MenACWY if applicable.

Complete Form 680 with your physician (2–4 weeks before)

Schedule a physician visit to complete any missing doses and have Form 680 signed. Indiana requires physician or authorized provider sign-off — parents cannot self-certify immunization records for school enrollment.

Submit to your school before the first day

Form 680 must be on file before school begins. Indiana allows a grace period of 20 days for students whose records are in progress — contact the school nurse if your child needs additional time and has a documented catch-up schedule.

6th grade: confirm Tdap booster and MenACWY

Before entering middle school, confirm both the Tdap booster and the first MenACWY dose are on your child's record. Missing either at grade transition is one of the most common reasons Indiana families face last-minute school enrollment delays.
