Under 6 CCR 1009-2 (Colorado School Entry Immunization Requirements), Colorado schools must collect a Certificate of Immunization for every student. The certificate must document dated dose records for each required vaccine group.

Students entering 6th grade require a Tdap booster and a meningococcal (MenACWY) dose. Colorado's required schedule is updated periodically by the Colorado Department of Public Health and Environment (CDPHE).
CIIS (Colorado Immunization Information System) is Colorado's statewide registry. Healthcare providers file immunization records electronically to CIIS. Parents may request a printed immunization history through their healthcare provider or their local county health department. A CIIS-generated record is accepted by Colorado schools as documentation of immunization history.

If CIIS doesn't have a complete record, bring existing immunization documents to your child's healthcare provider. They can verify dose dates and complete Colorado's Certificate of Immunization for school submission. The certificate requires a healthcare provider's signature or an authorized CIIS printout.
